PHQ-9 Depression Screening

This standardized questionnaire helps your provider understand how you've been feeling recently. It takes about 2 minutes.

Over the last 2 weeks, how often have you been bothered by any of the following problems?

2. Feeling down, depressed, or hopeless

3. Trouble falling or staying asleep, or sleeping too much

The PHQ-9 is a standardized, publicly available screening tool developed by Pfizer Inc. and colleagues; it is not a diagnosis on its own and your provider will review your results with you.
